If you have already scheduled a counseling appointment with a member of our team, we are grateful for the chance to serve you and those you care about. Please take the time to complete the New Client Forms above and bring them with you to your first visit. If you are unable to print them, please arrive to your appointment 20 minutes early to complete them before your first session.
